Attachments

The applicant of an unemployment benefit must give the fund all necessary information that is needed for granting and paying the benefit. The information the fund needs depends on whether you are completely unemployed, laid off, laid off for a shortened work week or laid off for shortened work hours, part-time employee or a person who works occasionally.

Salary certificate or a pay slip

As a rule, send the fund a salary certificate for 26 calendar weeks as an attachment to your application. We need the salary certificate so that we can define the salary upon which your daily allowance is based. If you have had multiple employments during the 26 weeks before your unemployment, you need to deliver us a salary certificate for each employment separately. Please notice that the employment must be one that can be counted towards the employee’s employment condition. The salary certificate has to be from the period, during which you have met the employment condition. If you do not have any weeks that are counted towards the employment condition (as a rule, at least an 18-hour work week) from 30.12.2013 onwards, you have to deliver the salary information of at least 34 calendar weeks.

The salary certificate must contain the following information:

  • All salaries you have earned during the period of the certificate. The salary certificate must not contain salaries you have earned outside the period in question. Salaries are reported according to the earning principle. This has to be taken into account especially when reporting production rewards, bonuses and other such portions of the salary. If there are unpaid salaries during the period of the certificate, information about them must be separately marked on the certificate.
  • Information about the date of salary payment.
  • Absences without pay. These periods that need to be reported include for example sickness, study leave and maternity, paternity and parenting leave.
  • Vacation bonus and leave allowance separately marked. Vacation bonus is paid at the end of the employment as a compensation for accumulated vacation time. Payments of leave allowance are based on the collective agreement.
  • The name and phone number of the employer’s contact person.
  • The signature of the employer’s contact person.

The last pay slip is not a sufficient report  on  the salary earned as it does not contain all the information needed.

Notice of redundancy 

According to the act on work agreements, the notice of redundancy must be delivered to the employee personally. If it is not possible, it can be delivered by mail or e-mail. The notice can thus be either written or oral, but by the employee’s request the employer must report the date of termination and the causes of the termination in written form. The fund also needs information on when the work agreement has been discontinued, to define the period of termination. There is no entitlement for unemployment benefits for the period when the employer is obliged to pay a salary for the termination period or other such benefit.

Contract of employment

You must deliver a contract of employment to the fund at least when your terminable employment has ended or you have been laid off temporarily from a part-time work or your working hours have been cut down based on a temporary lay-off. If there is no written contract, the fund needs a written report from the employer, where the most important terms of the employment are noted.

Notice of lay-off

According to the act on work contracts, the employer must notify the employee of a lay off at least 14 days before the beginning of the lay-off. The notice can be given personally, by mail or e-mail. The notice must state the cause of lay off, the date of its beginning and duration or estimated duration. To determine the period of a lay-off notice, the fund needs information on when the notice has been given.

Tax card

The fund receives the tax information of its old members from the tax authorities. Those who have joined during the year must deliver an original tax card to the fund. To make sure we operate with the correct tax rate, you should ask the tax authority for a new tax card for the earnings-related daily allowance. In this case the fund uses the income tax per cent according to the tax card. The tax card for the daily allowance must be attached as an original to the application for daily allowance.

The accounts for other income and benefits

If you get income during unemployment or you for example work part time, you should attach a certificate of your income to the application. If you are a part time entrepreneur or farmer, deliver your last verified tax information to the fund. If you have started a part time entrepreneurship, deliver an income tax note to the fund.

If you receive any social benefit, deliver a copy of the latest decision to the fund. Notify the fund of applying for pension or of an appeal. If you or your family receives home care supplement of a child under the age of three, attach information on whether your spouse takes care of the child to the decision.

Statement from the employment office

Register at the employment office on your first day of unemployment at latest. The labour politic statement will be transferred to the fund electronically.

Weekly working hour list

For employment condition and as a basis for the daily allowance, the fund can only take into consideration the weeks when your working hours have been at least 18 hours per week. Please note that on some fields (for example teaching and artistic work) also working weeks under 18 hours per week can be approved.

If you have done two or more part-time jobs, deliver a weekly working hour list from each employer, where your working hours have been stated. In addition, you must deliver the fund a weekly working hour list if your working hours have varied so that some weeks you work under 18 hours a week and some weeks you work over 18 hours per week.

Child supplement

The fund receives information of your children from the population register system. If there is an agreement made on child care and maintenance, deliver the agreement verified by the social committee or the decision made by the court to the fund.

Information predating the unemployment

If you have been on a maternity leave or on a care leave predating the unemployment and you have had income during those times, attach information of your employment and a salary certificate made by your employer to the application.
